The payer is rejecting with a message stating that either 1.) you need to have prior authorization and you did not send an authorization on the claim or 2.) the authorization that you supplied is not valid.
How to Fix
To fix this, you will want to make sure the authorization number is going out on the claims and it is a valid authorization number for the payer you are submitting to.
