The insurance company is rejecting stating that the date of birth you supplied for the Member (a.k.a the Subscriber, the Insured) is either not present, not valid, or not what they have on file for the member.
Some insurance companies are very strict on matching the date of birth of insured (member) that you send on the claim with the date of birth that they have on file for the member in their system. For this reason, we recommend that you make it common practice to collect the date of birth of the member (insured) on intake.
How to Fix?
The Member date of birth is entered on the insurance card for the patient/client in Therabill. Go to the edit client form for this patient. On the edit client form, under the Insurance Cards section, click the edit icon associated with this insurance card. On the edit insurance card form, enter a valid (and correct) date of birth in the birthdate field. Remember to save!
