The payer is rejecting because they have found the Zip code that you have provided for the service facility address to be invalid. Unless you have set up separate facility addresses under Admin->Member Info in Therabill, the facility address is most likely the same address that you have entered as your billing provider address.
- Make sure the Zip code that you have entered is numeric and 5 characters in length.
- Make sure the Zip code that you have entered matches the state that you supplied. That is, sometimes the zip code can be correct, but the two letter abbreviation for the state that you entered is not valid (or not valid for that Zip code).
- Make sure the Zip code you supplied matches what the payer has on file for this patient.
