What it is:
Box 11c is the name of the insurance plan or program of the insured. Some payers require an identification number of the primary insurer rather than the name in this field.
Therabill will use the value from the Plan/Program field that is in the Policy Info section of the Insurance Card to fill Box 11c.
On the top menu bar, select Clients / Client List. Click the blue pencil for the desired client. Click Add Card or the blue pencil of an existing insurance card. You will now see the Add/Edit Insurance Card form. In the Policy Info section, find the field labeled Plan/Program (see image to right). Enter the plan name or identification number of the primary insurer as required by the payer.
