What it is:
Box 21, ICD Ind, is used to identify the applicable ICD indicator to specify which version of ICD codes are being reported.
Box 21, Lines A through L, are used to indicate the sign, symptom, complaint, or condition of the patient relating to the service(s) on the claim. Up to 12 ICD-9-CM or ICD-10-CM diagnosis codes can be entered. Relate lines A - L to the lines of service in Box 24E by the letter of the line. Use the highest level of specificity. Do not provide any narrative description in this field.
Therabill will use the values from the Add/Edit Case dialog of the client information to populate the data for Box 21.
On the top menu bar, select Clients / Client List. Click the blue pencil for the desired client. You will now see the client information form. In the Diagnosis (Cases) section, click on the blue pencil for the correct case or click the green Add Case button. You will now see the Add/Edit Case dialog. Enter the ICD-9 diagnosis codes for this case.
You can also enter ICD-10 diagnosis codes, but these are only informational at this time. They are not yet required for use and will not be put on the claim.
You can also set up multiple diagnosis cases and select which one will be used to fill out Box 21 when you enter information for a specific session.
