While Therabill does maintain a list of insurance companies, along with their EDI Payer ID (kind of like their electronic address), we do not maintain a list of physical addresses for all insurance companies. If you would like to have the physical address of the insurance company show up at the top right of the CMS-1500 claim form, then you will first need to enter the physical address for the insurance company into Therabill. You can normally get this address from the insurance card that you received from your client/patient.
Adding the physical address of the insurance company in Therabill
You add the insurance company address on the insurance card in Therabill (see image to the right). At the bottom of the insurance card entry form, you will see a bar labeled Payor (Insurance Company) Address (see image to the right, red arrow). Initially, the address block is closed from view, simply put your mouse over the bar to display the insurance company address entry form.

IMPORTANT: If you enter an address, the address that you enter does go out on EDI (electronic) claims as the payor address. An incomplete address will result in an EDI rejection. Therefore, either leave the address completely blank or fill in all of the fields (street, zip, city and state). For example, do not enter the zip, city and state without entering the street address.
What will entering an insurance company address do?
If you enter an insurance company address, Therabill will put the address that you enter on the top right of your CMS-100 claim form when you create a claim to that insurance card. While the address does not route claims electronically (the payer ID is used for electronic claims routing to the payer), if you do enter an address, it does go out on the electronic claim. Therefore, if you do enter an address, please make sure it is completely entered. You must either enter all of the following fields: street, zip, city and state or leave them all blank. An incomplete address will result in an electronic (EDI) rejection.
A Default Address for the Insurance Company
You can create a default address for an insurance company in one of two ways.
- By checking the Make this the default for this payer when entering an address for an insurance company on the insurance card.
- By entering a default address in your insurance list. To get to your insurance list, put your mouse over Billing in the top navigation bar of Therabill and then click on Insurance List in the drop down menu. You will now see a list of your insurance companies. Click the Info & Settings link associated with the insurance company that you would like to add a default address to. In the settings form that opens up, click the Addresses tab at at the top.
What does a default insurance address do?
Creating a default insurance address will pre-fill the insurance company address on the add insurance card form whenever you are creating a new insurance card and you choose that insurance company as the insurance company on the insurance card. Creating a default insurance address will not change the address on any insurance cards for that insurance company that you have already created.
